Writing this book has been a labor of love—labor because it takes concentrated time and effort to write a book while also seeing patients, writing grants, lecturing, supervising, and serving as administrators; and love because this work reflects our combined 60-plus years of interest, knowledge, and experience in an area to which we are both passionately and deeply committed. It also reflects our strong beliefs regarding the value of integrating psychopharmacology and psychotherapy. We have had throughout our careers wonderful mentors and colleagues who have helped us refine our thinking, ask better questions, and pursue deeper and more accurate answers. They include Dr. Michael Goldstein, who is no longer with us, as well as Drs. Kay Jamison, Connie Hammen, Lori Altshuler, Ira Lesser, Mark Frye, Ellen Frank, David Kupfer, David Axelson, Kiki Chang, Chris Schneck, and Boris Birmaher.
We are indebted to our chairman, Professor Peter Whybrow, for his leadership in bipolar disorder research and scholarship for two generations. David J. Miklowitz has been fortunate to have had wonderful graduate students, postdoctoral fellows, and psychiatric colleagues who have been central in his clinical work and research, both at UCLA and formerly at the University of Colorado. For Michael J. Gitlin, over 30 years of teaching residents at UCLA, especially in the Mood Disorders Clinic, have been invaluable in sharpening his thinking about this fascinating and troubling disorder. Of course, we are both grateful to have met and begun working together at UCLA over 30 years ago, and for the wonderful friendship that has emerged.
